But a small kitchen staple most people already have on hand can also blunt how sharply blood sugar rises after a meal, and the research behind it goes back two decades. Why Refined Carbs Cause the Spike
White bread, soda, and other refined carbohydrates break down into glucose fast, so blood sugar rises quickly after eating and then crashes soon after. Repeated spikes like this, meal after meal, are linked to growing insulin resistance over time — the body's cells gradually respond less to insulin, so the pancreas has to work harder to keep blood sugar in range.
What the Apple Cider Vinegar Research Found
In a 2004 study published in Diabetes Care, researchers at Arizona State University gave 29 participants — some with normal insulin sensitivity, some with insulin resistance, and some with type 2 diabetes — 20 grams of apple cider vinegar diluted in 40 milliliters of water before a meal containing 87 grams of carbohydrates. In the insulin-resistant group, insulin sensitivity improved by 34% compared to placebo; in the type 2 diabetes group, it improved by 19%. The effect showed up after a single dose, not weeks of use — acetic acid appears to reduce how much insulin the body needs to process a starchy meal.

A Bedtime Dose Made a Difference Too
A follow-up pilot study from the same research group, published in Diabetes Care in 2007, had adults with well-controlled type 2 diabetes take 2 tablespoons of vinegar at bedtime. The next morning, fasting blood glucose was about 4% lower on average — and up to 6% lower in participants whose fasting glucose started above 130 mg/dL. Researchers pointed to acetic acid's effect on the enzymes involved in carbohydrate digestion as the likely mechanism. Both studies were small and short, and each measured single readings rather than long-term control, so they point to a real effect without settling how much it matters over months or years.
How to Actually Use It
- Always dilute it — Straight vinegar is acidic enough to wear down tooth enamel and irritate the throat over time. Stick to roughly 1–2 tablespoons mixed into a full glass of water.
- Keep the daily total modest — The lead researcher on the original study, Carol Johnston, has recommended staying at or under 4 tablespoons of vinegar a day.
- Time it around meals — Taking it shortly before a carb-heavy meal, or before bed, matches how the studies above were designed.
- Check with a doctor if you're on insulin or other blood sugar-lowering medication, since combining the two could lower blood sugar more than expected.

Alongside the Plate, Not Instead of It
Vinegar isn't a substitute for diabetes treatment, and it won't undo a diet built around refined carbs. But the research suggests it's a small, low-cost habit with real evidence behind it — one that works alongside, not instead of, regular exercise and a balanced plate.
